If this problem is occurring locally (on your computer as opposed to dictating over a network) in Select-&-Say applications such as Microsoft Word, WordPerfect, DragonPad etc. then we think it may be related to a corrupted user profile. The last time we ran into this problem, creating a new user resolved the issue. Depending on what flavor and version of NaturallySpeaking you are using, you may be able to export and import your vocabulary and commands into your new user profile.
