		<description><![CDATA[ &lt;p&gt;I am a doc for a large academic practice soon to switch to EPIC EMR. The deployment is using a thin client via Citrix. &amp;nbsp;Having already spent a **breathtaking** amount of money on this EMR, the institution is not providing any speech recognition support. So, I&apos;m on my own...but I&apos;m okay with that because they&apos;d probably just give me something even more klugey than EPIC...grrr - don&apos;t get me started. &amp;nbsp;&lt;/p&gt;
&lt;p&gt;I&apos;m planning to invest $2-3k of discretionary money to do this right and buy myself some more sleep time. &amp;nbsp;So what hardware and software would work best to start from scratch? &amp;nbsp;&lt;/p&gt;
&lt;p&gt;Experience: I have used DNS 9.5 for years. &amp;nbsp;Hey, stop laughing...it works just fine given the time I invested in training, but only at my desk in a quiet office with headset. &amp;nbsp;I&apos;ve been looking to upgrade hardware and software anyway.&lt;/p&gt;
&lt;p&gt;Environment: I have to move around to different rooms, different sites, and do the note completion in the exam room. So most practical is a laptop/ultrabook, preferably with touchscreen, or ideally tablet. &amp;nbsp;Ambient noise is not too bad, but I assume I&apos;ll need a microphone too, maybe needing a usb dongle. Mostly I want to be able to face a patient without looking like a borg and be able to convey that I care more about listening to them than making my computer work.&lt;/p&gt;
&lt;p&gt;Workaround-ability: Browsing the forum, it looks like dictating to Notepad with scripted transfer to the active EPIC field is probably the best option, but correct me if I&apos;m wrong.&lt;/p&gt;]]></description>

		<description><![CDATA[ &lt;p&gt;I&apos;m using Dragon Medical Enterprise version 10.1.&amp;nbsp; Since our IT department took over maintenence and updates etc. of Dragon the system has been performing very poorly.&amp;nbsp; I&apos;m am using Dragon Professional 12.5 with Knowbrainer Medical we great success with excellent accuracy on a 1st generation i7 acer laptop with 8MB of RAM.&amp;nbsp; I know that specific release dates are not made&amp;nbsp;public.&amp;nbsp; I&apos;m trying to plan for a change in EMR systems that will happen in Feb., 2014.&amp;nbsp; Anyone venture to quest when the Medical version will be upgraded with similar features (e.g., best Match IV)?&lt;/p&gt;

		<description><![CDATA[ &lt;p&gt;I have read elsewhere on this board that SpeechMagic&amp;nbsp;is&amp;nbsp;an &quot;SDK for speech enabling and integration of Medical applications. It uses the Dragon engine, but by itself it is not a speech recognition app.&quot;&lt;/p&gt;
&lt;p&gt;Are there comparisons for performance of Dragon in PowerScribe360 versus current version of SpeechMagic contained in other&amp;nbsp;speech recognition products.&lt;/p&gt;

I dictate mostly into DragonPad, and use a script (&quot;send text&quot;), that I think I found in this forum to transfer the content from DragonPad into the currently open window of the EHR. The question, should I loaded dragon first, or should I have a EHR open, and then load dragon. By the way, I have dragon set so that it is somewhat loaded (if not open), because the dragon logo is already present in the bar at the bottom of the screen. When I click to open Dragon it still asks which profile to activate.&lt;/p&gt;]]></description>

		<description><![CDATA[ &lt;p&gt;I have an Ophthalmologist that is trying to dictate &quot;OU&quot; (owe you) and have it come out as &quot;OU,&quot; NOT each eye or both eyes. Sometimes it comes out as OU, sometimes it it comes out as each eye or both eyes. Things I tried: With OU, OD, and OS, I made sure they were in the vocab. I also took out of the vocab the phrase &quot;left eye&quot; and &quot;right eye,&quot; but &quot;both eyes&quot; and &quot;each eye&quot; were not in there to delete. I also checked the formatting options and didn&apos;t see anything in there. He is using DMPE.&lt;/p&gt;

&lt;p&gt;When&amp;nbsp;dictating&amp;nbsp;into any applications running&amp;nbsp;via Citrix, Epic for example, the phrases are not being transcribed completely into the EHR system. It stops in the middle of the sentence and it happens pretty consistently. For example:&lt;/p&gt;
&lt;p&gt;Input:&amp;nbsp;The patient has a large wound&amp;nbsp;on the plantar aspect of her left forefoot area.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Output: The patient has a large wound&lt;/p&gt;
&lt;p&gt;Dictating&amp;nbsp;using the Dictation Box works like a charm, but the&amp;nbsp;ideal way is to just dictate directly into any app that runs via Citrix.&amp;nbsp;I&apos;d like to know if this is a known issue and if so what others have done to resolve it. &lt;p&gt;2. Another time, during dictation, it switched to &quot;individual character mode&quot;&lt;/p&gt;
&lt;p&gt;3. Quite frequently,&amp;nbsp;one will be dictating and Lotus Notes will open and create a new message, and the dictation text will be displayed in the &quot;To:&quot; field.&lt;/p&gt;
&lt;p&gt;We are using a html based EMR developed specifically for the Phoenix Fire Department.&lt;/p&gt;
&lt;p&gt;Originally, the profiles were all upgrades from 9, but when this specific doc (the one with the most issues) had all these problems, i had him start a brand new profile and retrain.&lt;/p&gt;
&lt;p&gt;Even with a new user profile he is still having similar issues.&lt;/p&gt;

		<description><![CDATA[ &lt;p&gt;We have a dr. her that dictates A LOT. &amp;nbsp;He has problems frequently, but we don&apos;t know what he is doing to create these issues. &amp;nbsp;We have several other drs using the product and ever call us (IT). &amp;nbsp;&lt;/p&gt;
&lt;p&gt;Here is the current issue.&lt;/p&gt;
&lt;p&gt;On the Attached: &amp;nbsp;&lt;/p&gt;
&lt;p&gt;The first red arrow. &amp;nbsp;We will ask | &amp;nbsp;the curser is after the ask. &amp;nbsp;He dictates &quot;The DME to address&quot; and it puts it at the end of the paragraph, and it ALSO changed the R in Return to an S. &amp;nbsp;&lt;/p&gt;
&lt;p&gt;Now, he says this happens 10-20 times a day. &amp;nbsp;The dictated sentance doesn&apos;t always go to the end of the paragraph, it is random. &amp;nbsp;&lt;/p&gt;

		<description><![CDATA[ &lt;p&gt;We are transferring to an EMR which uses Word 2010 to generate notes from templates. &amp;nbsp;So when you create a note from the program Word opens with the appropriate template &amp;nbsp;These are then converted to a signed PDF which is stored in the patient record. &amp;nbsp;It&apos;s nice because it seems to play well with DNS 12 professional. &amp;nbsp;It is a lesser known EMR at this point so no greyed out boxes. &amp;nbsp;The problem is working within the templates there are grey boxes for narratives. &amp;nbsp;As long as the narrative is a single utterance without a pause it is in the document. &amp;nbsp;However, if I pause during the narrative anything before that pause is overwritten and the latter part of the narrative is there provided I don&apos;t pause again-if I do it is again overwritten. &amp;nbsp;I have a message in to the EMR &amp;nbsp;IT guys. &amp;nbsp;I just wanted to make sure it wasn&apos;t a Word or DNS setting issue. &amp;nbsp;I do have to enable editing to complete these templates - which I do.&lt;/p&gt;

		<description><![CDATA[ &lt;p&gt;DNS v10.1 Medical installed on a Asus&amp;nbsp;laptop running Win 7 64K with&amp;nbsp;4 GB of RAM and an i5 2.27 GHz processor.&lt;br /&gt;&lt;br /&gt;The problem is this - whenever I get to the&amp;nbsp;&quot;Adjust Your Microphone: Volume Check&quot; screen, click the&amp;nbsp;&quot;Start Volume Check&quot; button and speak for about 5-7 seconds&amp;nbsp;the entire laptop locks up.&amp;nbsp; Hard.&amp;nbsp; So hard that&amp;nbsp;I can&apos;t even CTRL-ALT-DELETE to try and end the task.&amp;nbsp; No amount of waiting unlocks the laptop and I am forced to power down via a hard power off.&lt;br /&gt;&lt;br /&gt;I am connecting&amp;nbsp;an Andrea headset with a&amp;nbsp;3.5mm jack; the laptop itself&amp;nbsp;has a microphone embebed in the bezel at the top of the display to the right of the webcam, and best I can tell when I plug the headset in via Mic In, the imbded mic&amp;nbsp;turns off, and as soon as I unplug the headset&amp;nbsp;the imbeded&amp;nbsp;mic turns back on.&amp;nbsp; The system tray displays a popup when I plug and unplug the headset.&lt;br /&gt;&lt;br /&gt;What I&apos;ve tried so far:&lt;br /&gt;&lt;br /&gt;Uninstalling Dragon using the Dragon Removal app and reinstalling Dragon&lt;br /&gt;Using a&amp;nbsp;second Andrea headset&lt;br /&gt;Using a Speech Mike III&lt;br /&gt;Creating a new user&lt;br /&gt;&lt;br /&gt;After all of that, I still have the same exact problem when I get to the Volume Check screen.&lt;br /&gt;&lt;br /&gt;I have never seen this before with any&amp;nbsp;of my DNS Medical installs (close to 20).&amp;nbsp; I have attached the Dragon log in hopes that you guys can help.&lt;br /&gt;&lt;br /&gt;Let me know if you need additional technical information - thanks in advance!&lt;/p&gt;]]></description>

&lt;p&gt;We have a citrix hosted EMR that we have been trying to get Dragon to work with and it has been a bumpy road. We finally bought new PowerMicII and have the physicians using the dictation box with hotkeys, which has been a HUGE improvement. However, I have one doctor that Dragon keeps bumping out of the dictation box. For example, he pulls up the box with the powermic hotkey and starts to dictate but the preview box isn&apos;t by the dictation box instead its down by the system tray and never inputs the text into the box. It also tries to read this input as commands. No other doctor is seeing this problem and all have the same setup, I have sat for hours looking at settings, can someone help before I go crazy!! &lt;p&gt;From what I can find about Entrada voice recognition is recently been available in 2010. It does voice dictation. Does anybody have any experience.&lt;/p&gt;

&lt;p&gt;I have 4 providers that were using DMPE along with their EMR with no problems.&lt;/p&gt;
&lt;p&gt;Then, their EMR trainer went in and updated their Dragon to the 11.00.300.530 and had them read some training paragraph, and now they are experiencing latency, decreased accuracy, non-functioning commands, and the like.&lt;/p&gt;
&lt;p&gt;Any information/input would be greatly appreciated!&lt;/p&gt;

		<description><![CDATA[ &lt;p&gt;been using DNS for years. &amp;nbsp;now on my 4th or 5th version. &amp;nbsp;upgraded, scratch that, i uninstalled the old, and went with the DNS Medical in June 2012, creating a brand new profile as I have seen recommended on here previously. &amp;nbsp;&lt;/p&gt;
&lt;p&gt;overall, been quite happy with it, but one major problem. &amp;nbsp;my recognition accuracy drops significantly right after scheduled acoustic optimization occurs. &amp;nbsp;Typically, AO is scheduled to run once a month over the weekend when the office is closed, and then I get a rude awakening come Monday morning when trying to use the system. &amp;nbsp;Finally, I have disabled the acoustic optimizer, and plan to leave it off until this problem is rectified. &amp;nbsp;I have to make corrections in practically every sentence, and it takes almost a couple of weeks for things to get back near normal.&lt;/p&gt;
&lt;p&gt;I do an acoustic and microphone check regularly.&lt;/p&gt;
&lt;p&gt;Any thoughts on what might be causing this?&lt;/p&gt;

&lt;p&gt;1. Does KB 11D work with DNS 10.1 medical?&amp;nbsp;&lt;/p&gt;
&lt;p&gt;2. If so, can I just ask IT at the hospital to load it onto my profile workspace? (I have an extra license not in use for KB).&lt;/p&gt;
&lt;p&gt;3. What about KB 12 - would an upgrade be useful for use with DNS 10.1?&lt;/p&gt;

		<description><![CDATA[ &lt;p&gt;Hospital just implemented DNS 10.1 enterprise.&amp;nbsp; IT told me they could import my existing&amp;nbsp;vocabulary and commands (DNS Small Practice 11).&amp;nbsp; I sent them the DAT and txt files.&amp;nbsp; They then told me I needed to give them a complete copy of my user file.&amp;nbsp;&amp;nbsp; I did. They did the import. After that I could not log in to DNS.&amp;nbsp; IT told me they had been mistaken and it is not possible to import vocabulary or commands from DNS 11 to DNS 10.1 enterprise.&lt;/p&gt;

		<description><![CDATA[ &lt;p&gt;When using Dragon Medical Practice Edition v11 with eClinical Works 9&lt;/p&gt;
&lt;p&gt;Each time a new field is selected and dictation begins, the first sentence (or first word, it doesn&apos;t matter how much you say) takes at least 15 seconds to actually type out what was said. After that initial dictation, it becomes instant and works like normal. It will continue to work like normal until a new field is selected. When a new field is selected (just by clicking the mouse into the new field), again the initial statement isn&apos;t registered for at least 15 seconds.&lt;/p&gt;
&lt;p&gt;-When dictating into MS Word or any other application there is no initial lag.&lt;br /&gt;-When I select &quot;DragonPad&quot; from the Dragon toolbar, there is no lag when dictating into that.&lt;br /&gt;-When I select &quot;Dictation Box&quot; from the Dragon toolbar, there is a 10 second lag when dictating into that.&lt;/p&gt;
&lt;p&gt;I spoke with Dragon Support (Know Brainer) and they did not feel the issue was with the Dragon software or eClinical Works since it works in Word and they have not seen this issue before working between Dragon and eCW. They feel this is an issue with the network.&lt;/p&gt;
&lt;p&gt;This issue is present on all 3 PCs with Dragon installed. I have tried disabling antivirus, but that didn&apos;t help.&lt;/p&gt;
&lt;p&gt;&amp;nbsp;&lt;/p&gt;

		<description><![CDATA[ Bought Dragon 11.0 and understood that it could only be used on the &amp;quot;fat side&amp;quot; ( without signing in to the server). There are issues, probably related to our ISP and our server not being in our current building, that cause slowed navigation on Fat, so it takes some of the joy out of the VERY fast, VERY accurate dictation I have been able to do.&amp;nbsp; So one day, I was on the thin client, and my Dragon Tool Bar, open on fat, popped up where I could see it on thin. I went ahead and tried to dictate. It did about as well as Version 7.0. Recognition not as good. Periods in the wrong places. Sometimes stopped before the end of the sentence. But: 1) I don&amp;#39;t understand how this could happen; and 2) I would love to make it work better on thin.... ideas? ]]></description>

		<description><![CDATA[ &lt;p&gt;Newbie, &lt;/p&gt;&lt;p&gt;I&amp;#39;m a PA for several cardiologists and have a lot of problems with the physicians routing, they don&amp;#39;t particularly like how many steps it takes. &lt;/p&gt;&lt;p&gt;I tried a macro but that lasted all of 1 day and then did not produce the next. &lt;/p&gt;&lt;p&gt;I keep reading on the forum about scripting.&amp;nbsp; I have never script before or know the first thing about it but have read a lot on the subject, it&amp;nbsp;sounds like it might be the answer to my problem.&amp;nbsp; We work in EPIC 2009 and have Dragon NaturallySpeaking 10.1.&amp;nbsp;&amp;nbsp;&amp;nbsp; &lt;/p&gt;&lt;p&gt;Several of the physicians will not touch Dragon until we get this resolved.&amp;nbsp;&amp;nbsp;If&amp;nbsp;you guys could help me with this problem it would make my year.&amp;nbsp; &lt;/p&gt;&lt;p&gt;I no this is the Knowbrainer forum but this is beyond me.&amp;nbsp;&amp;nbsp; &lt;/p&gt;&lt;p&gt;Thanks ~ Gwen&amp;nbsp;&lt;/p&gt;&lt;p&gt;&amp;nbsp;&lt;/p&gt;&lt;p&gt;&amp;nbsp;&lt;/p&gt;]]></description>

So far I&amp;#39;ve been dealing mostly with German-speaking clients. Now I am dealing with a medical company where English is also widely spoken.&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;Due to the lack of experience of speech recognition vendors in Switzerland with English-speaking clients I find myself in the position of having to seek help here: what are your experiences with the American or British Dragon Medical editions?&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;Are the built-in vocabularies actually accurate? Do you have to do a lot of training or is the accuracy with the medical editions acceptable from the get go? Note that the Dragon Medical editions for German are pretty much unusable for the common physician without extensive customization.&amp;nbsp;&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;Or do you go the same route as in Europe and buy the vocabularies from specialists in vocabulary building?&amp;nbsp;&lt;/div&gt;&lt;div&gt;(Lunis, I have seen that you offer a KnowBrainer medical version. 		<description><![CDATA[ &lt;p&gt;Not sure if this forum is only for Support issues regarding DNS for windows. &amp;nbsp; I use both Dragon NaturallySpeaking version 10 medical for Windows as well as MacSpeech Dictate medical for iMac. 		<description><![CDATA[ &lt;p&gt;I am currently using dragon medical 11 to dictate into practice partner from Mckesson.&amp;nbsp; My practice partner is over a&amp;nbsp;Citrix server.&amp;nbsp; I notice a problem only when dictating into the EMR.&amp;nbsp; My dictations become garbled at times.&amp;nbsp; If I look at the dictation box, there is supposedly is no problem.&amp;nbsp; I do not notice this when I dictate into dragon pad or other programs.&lt;br /&gt;&lt;br /&gt;Here is an example:&lt;br /&gt;&lt;br /&gt;Instead of, &amp;quot;significant valvular heart lesions&amp;quot;&lt;br /&gt;&lt;br /&gt;I get, &amp;quot;significant vlurt lesions&amp;quot;&lt;/p&gt;&lt;p&gt;&amp;nbsp;As you can see, I do not think this is a problem with misrecognition .&amp;nbsp; Of the word, it seems to garble the words together&lt;/p&gt;&lt;p&gt;I noticed this more commonly when I dictate longer passages.&amp;nbsp; This is unfortunate, because this is when dragon makes my life easier.&amp;nbsp; I always thought this was a problem with my connection to Citrix, but I wanted to make sure it is not something else.&amp;nbsp; Any help will be appreciated.&lt;/p&gt;]]></description>

		<description><![CDATA[ &lt;p&gt;I am a disabled physician using Dragon for navigation in our EHR as well as for dictation. I open results queues, sign, print, etc all with Dragon. As you know, one can navigate in Yahoo!, Sermo and numerous other sites and programs, including my EHR, iKnowMed, via Dragon Medical 8, 9, 10.. However, DMP 11.5 represents an unpleasant set back-I can navigate in Microsoft office apps and that&amp;#39;s about it. Now, several months after the release, I have not found any more applications with &amp;quot;full text control&amp;quot;. &amp;nbsp;It looks like I&amp;#39;ll be rolling back to 10.1 -- for now? &amp;nbsp;forever? Are there any rumors out there? 		<description><![CDATA[ I have been having problems when dictating into WordPerfect with the .dra files. I am using WordPerfect X5. Many times while I am dictating in WordPerfect, I will close the file and save it and come back to it at a later time for further dictation. I may do this several times during the day since I am using it for medical notes. If I do not correct the dictation at the time that I dictate I have noticed that if I do several dictations and close and save the file that many times I cannot go back and play back the sound file at all. Many times the only thing that will be saved is the last sound file. In the options setting I have the &amp;quot;save recorded dictation with document &amp;quot;set to always&amp;quot;. I do not have anything set in the options file on that data tab under the advanced settings. Is this the mistake that I am making? I notice under the advanced button under the data tab that one of the options is &amp;quot;always preserve wave data&amp;quot;. Should I be setting that with a check mark? Any input would be welcomed. I am using Windows 7 ultimate edition with all service packs installed.]]></description>

We organize files by case, not patient. 		<description><![CDATA[ &lt;p&gt;Our medical group is beginning implementation of a non-blacklisted EMR. &amp;nbsp;We have 3 or so docs who would like to use voice recognition. &amp;nbsp;&lt;br /&gt;Thanks to all of the forum contributors - I&amp;#39;ve perused the archives for hours and had most of my questions answered. &amp;nbsp;Still have a few Q&amp;#39;s though.&lt;/p&gt;&lt;p&gt;Q1: My reading of the forum has led me to believe that DNS Pro + Knowbrainer provides more advanced speech recognition (because it&amp;#39;s on the 11.5 version) than the current Dragon Medical Practice Edition and at a lower cost. &amp;nbsp;Are their any downsides to the DNS Pro + Knowbrainer approach when compared to DMPE for use with a non-blacklisted EMR?&lt;/p&gt;&lt;p&gt;Q2: I&amp;#39;ve read that DNS Pro can be used on up to 5 machines. &amp;nbsp;Is this one user on up to 5 machines (ie. does each user need a separate license) or for multiple users (even if more than 5) as long as its limited to use on 5 machines?&lt;/p&gt;&lt;p&gt;Q2:&amp;nbsp;&amp;nbsp;Does the Medical Practice Edition also allow use on up to 5 machines or does it require per-user licensing?&lt;/p&gt;&lt;p&gt;Thanks in advance for your help.&lt;/p&gt;&lt;p&gt;&amp;nbsp;&lt;/p&gt;&lt;p&gt;Andrew&lt;/p&gt;&lt;p&gt;Not currently using speech recognition software&amp;nbsp;&lt;/p&gt;]]></description>

		<description><![CDATA[ &lt;p&gt;I have the left custom button set to &amp;#39;Previous field&amp;#39; and the right button to &amp;#39;Next field&amp;#39;.&amp;nbsp; For no apparent reason these buttons no longer work.&amp;nbsp; I have 3 PowerMics so I tried another one but still no joy.&lt;/p&gt;&lt;p&gt;It may not have any bearing on this issue but I searched all commands and cannot find the Previous field or Next field commands in KB or DNS.&lt;/p&gt;&lt;p&gt;I have all of the other buttons set to different commands and they all work as usual.&amp;nbsp; I tried to switch the Previous/Next field commands to a different button but they still did not work.&lt;/p&gt;&lt;p&gt;I am using the same forms with the same format for fields (bracketed numbers) and this has worked for years.&lt;/p&gt;&lt;p&gt;Solution?&lt;/p&gt;&lt;p&gt;EDIT TO ADD:&amp;nbsp; I am using DNS Medical 10.1/KB 2008&lt;/p&gt;]]></description>
