Assuming you are getting hit by Dragon freezing, while dictating into a local Dragon friendly environment like Microsoft Word… Try dictating a phrase followed by saying play that back. You are listening for interference. From everything you say, it sounds like Dragon's buffer is getting overflowed with internal noise like RF interference, microwaves etc.
PS: If you dictate very quickly, you might also consider doubling your Disk space reserved for playback option which you will find in the DragonBar Option/Data tab. Dragon's 100 Mb setting is often too low for transcriptionists, radiologists, pathologists, lawyers and most importantly us

OK, the plot thickens. Playback was fine. I then powered off the nearby desktop computer and external speakers without change. I changed microphones without difference. Last night noted that entire words (once even 2 words) were occasionally dropped as well, sometimes repetitively with one try after another. Recognition history and playback failed to show those words dictated. Continued to have dictation freezing. Changing USB ports on the computer for the external soundcard made no difference.This is all dictating directly into Notepad without going through Citrix. This is not at all the pattern of problems that I recognize from corrupted user profiles in the past. Additionally, after noting problem dropping words last night and further reflection, it occured to me that this was only happening while dictating at home at night and not at the office (also with DMPE 4.3). So now going to change external soundcards and see what tonight's dictation yields. The suspect external soundcard is 12 years old. If that doesn't fix the problem, will try dictating using my other home desktop. So I am clearly beginning to suspect that DMPE 4.3 may not be the issue. Will report back.

And further info:
Using different (new) external soundcard did not help. Using my other home desktop (also has DMPE 4.3) yielded same issue. I demonstrated this over and over again by on both home desktops. It kept dropping words (in this case "dermatologist", so not an unusual word). Then tried exactly same sequence of words at work and it worked reproducibly and repetitively, but never at home. The only difference is that since I do not have administrative privileges at work, had to have an IT person remote onto my work desktop and install DMPE 4.3. Since I figured that the IT person would be loath to click on a link they were not familar with, I had downloaded DMPE 4.3 from link (supplied by Lunis) at home to my desktop, then transferred to an external hard drive. I then took the external hard drive to the office, and the IT person installed from that external hard drive. So I suspect there may be some Dragon corruption going on directly installing from link (which I did from home) versus installing from hard drive. This is the only difference as microphones and external soundcards are otherwise the same. I will test by uninstalling DMPE 4.3 at home and reinstalling via external hard drive rather than directly from link. Does any of this make sense?

See above. Additionally will try with MMODAL at home tonight. My guess, it will work fine (further eliminating thoughts of interfering environmental issues).

MMODAL worked at home without dropped words. Once again DMPE 4.3 dropped words at home. Uninstalled DMPE and reinstalled from external harddrive as opposed to directly from link, but unfortunately, this made no difference. So I am at a loss how to deal with this very frustrating problem.

This is more of a wild guess, but you may have been hit by auto gain, which wasn't around and wasn't even a glint in the mother's eye during version 10, but trended to become an issue after it had been introduced some time after Captain Kirk resigned from Starfleet Academy.

Thank you for your help. So I just turned off auto-gain, did the short microphone training requested to calibrate the microphone, then as before, dictated the same sequence of 8 doctor names, followed by the type of doctor, such as:
Dr. Wilke, computerologist; Dr. Orcutt, KnowBrainerologist; etc.
Of the 8 doctors followed by the types of docs, the same pattern followed. It got all of the 8 names, but totally dropped the descriptive words: ophthalmologist, oncologist, and dermatologist. Needless to say, these are common doc descriptions and Dragon would be expected to get these easily.To be clear, I am not talking about a misrecognition, but rather entirely dropped/missing words. It is not as if it was a little odd-ball thing such as immunopathologist or neuro-otololaryngologist. So it was a very good idea, but no such luck. Dropping 3 words in this short dictation is very significant and intolerable. And to edit/correct my previous info above, I had not appreciated that it was also doing the same at the office.

You get a gold star for your creative thought/help, BUT still 2 of the eight docs specialties were still dropped (ophthalmologist and dermatologist). So variably 8 - 12.5% of words are dropped in the example given.This really is intolerable. Nuance has an ethical responsibility to FIX THIS, just like Trump has one to resign! But don't hold your breath for either.

Since I work at the ethics department of Nuance, let me see if can help.
Mistakenly, obviously, I had assumed that the problem is with Dragon dropping words at the beginning of an utterance, in combination with latency when resuming dictation. Indeed, a tendency to drop words every now and then seems to have emerged with the advent of version 15, but dropping multi-syllable words is kind of odd really.
Can we try a little experiment, if the issue is replicable? Would you mind dictating a sample demonstrating the issue into DragonPad, saving the dictation as a DRA file, and attaching the DRA file, along with a copy of the recognition history, saved with details?

Please note:
1. The above is very, very reproducible. This is demonstrated dictating into either Notepad or Dragonpad directly, without Citrix as a variable. 2. Seems like the same words are dropped each time. Training those words does not help, but this should not be necessary with an Internal Medicine vocabulary. 3. Using TwistMike microphone. Switching to BOOM O gives same result of dropping words, no better, no worse. 4. Changing USB ports is not helpful. 5. Changing computers is not helpful. 6. Changing user profiles is not helpful. 7. Uninstallining and reinstalling DMPE 4.3 is not helpful. 8. Changing external soundcards is not helpful. 9. Turning auto-gain either off or on is not helpful 10. Did not occur with Dragon Medical v. 10.1. Currently using DMPE 4.3 with Internal Medicine vocabulary. 11. Trying Eric's suggestion of low background music does not help. 12. The problem does not occur with first dictated words.
The only other thing that came to mind as I was summarizing this list was trying a General Medical vocabulary (which is what I used with Dragon Medical v. 10.1, rather than the Internal Medicine vocabulary that I am using with DMPE 4.3). In terms of thinking of other variables and possibilities, I thought, maybe the Internal Medicine vocabulary is corrupted???

This may sound weird, but try running an audio program in the background (like Pandora). I experience similar symptoms if I don't have Pandora playing music in the background. I think Windows 10 causes timeouts or pauses of the audio system if it's not constantly being used. If I don't have Pandora in the background, when I dictate it misses words (especially after pauses) and the first second or two of a dictation is cut off.

Still on the subject of Auto Gain Control. In the neutered embedded Dragon that I have to use, AGC is entirely a "Windows phenomenon". For DMPE, it sounds as though the is a tickbox inside the Program itself that let's a person toggle AGC within Dragon.
However, if microphone volume was already very low in windows - for whatever reason, toggling off AGC inside Dragon may not have any immediate effect in bringing the gain back up.
So just for the sake of retracing the steps, I suppose the "Input Volume", i.e., gain, for whichever mic you are using, is at maximum within Windows 10 sound control interface?
I got burned by that a number of times as even when DME was not running, Windows seems to have a surreptitious way to do AGC.
Do you have Skype or similar on your home computers? FWIK, they do their own AGC as well - usually without giving user any prompt or clue at all that it has happened.

Well, I was wrong. It is not just the Internal Medicine vocabulary that is corrupted. The same problem with dropping VERY predicable words also occurs with the Medicine (general) vocabulary for the following words.(other speciality words work fine):
allergist dermatologist endocrinologist gynecologist hematologist ophthalmologist orthopedist pathologist physiatrist podiatrist psychologist rheumatologist
But I have figured out an interesting workaround that sheds light on the problem. Transcription yields failure of dictation of the above words reliably and reproducibly. However, if one makes a Text & Graphics command, for example named "Dermatologist" to display "dermatologist" (all wihout quotes), it works reliably and reproducibly. This must be a hint into the underlining cause.

Out of curiosity, I just tried the list in my UK English General vocabulary, and oddly enough, except for one, they are all there, some of them being spelled a little differently, however.
But, it isn't any wonder that when entering one of these words as command name, it will be recognised right away even if it isn't in the vocabulary because command names don't have to be part of the vocabulary. And if the pronunciation is easy to guess from the spelling, Dragon doesn't have a problem with it, even without training.

Interestingly, I checked and all of those words are in the vocabulary (not added by me), but for whatever strange reason are not recognized, and thus "dropped" when dictated.
